Job summary

AA Healthcare Management is seeking a skilled MDS Coordinator to join our nursing leadership team. The role involves assessing residents, determining health status, care plans, and intake procedures in alignment with state and federal standards.

You will spearhead Medicare/Medicaid MDS processes, ensure timely and accurate completion, and work with the interdisciplinary team to address restorative program needs and compliance. Wisconsin RN licensure is required or in good standing.

Qualifications

• Minimum 1 year experience as an MDS Coordinator.
• Valid nursing license in Wisconsin in good standing.
• RN preferred.
• Proficient with computers, Outlook and Office.
• PCC knowledge and experience preferred.

Responsibilities

• Understand MDS 3.0 terminology and the entire MDS process for reimbursement.
• Execute accurate completion of Medicare/Medicaid MDS processes to ensure appropriate reimbursement.
• Use strong assessment skills and be detail-oriented with MDS completion.
• Coordinate with the Interdisciplinary Team to ensure MDS compliance and discuss programming for patient needs.
