Job summary

Carnegie Village Rehabilitation & Health Care Center is seeking an experienced MDS Coordinator (RN) to manage RAI and care management from admission through discharge, ensuring compliance with Federal, State and Certification guidelines. The role requires current RN or LPN licensure in Missouri, with LTC/SNF/AL/MC experience highly preferred.

Qualifications

• RN or LPN licensure required
• Prior MDS experience highly preferred
• Experience in LTC/SNF/AL/MC setting preferred
• Excellent written and oral communication skills
• Strong knowledge of Case-Mix and Medicare PPS processes
• Understanding OBRA regulations and Quality Indicator processes

Responsibilities

• Coordinate RAI and care management processes from admission to discharge.
• Ensure compliance with Federal, State and Certification guidelines.
• Assist with information systems operations and staff education as directed by leadership.
• Maintain accurate data tracking for admissions and discharges.
