Job summary

Acts Retirement-Life Communities, Inc. is seeking a qualified MDS/Care Coordinator (RN) for our skilled nursing neighborhood. You will coordinate care for residents and admissions, ensure MDS compliance, and lead care planning across disciplines.

Qualifications

• Current State RN license.
• Minimum of two years' MDS experience in a long-term care setting.
• Current or eligible for CPR certification.

Responsibilities

• Coordinate care for existing residents and new admissions in the skilled care community.
• Perform initial and routine MDS assessments and coordinate with the interdisciplinary team.
• Ensure compliance and completion of the MDS and oversee care planning.
• Serve as liaison between community, payer, and corporate office.
• Coordinate restorative nursing goals and educate staff on implementation.
