Job summary

Regency Integrated Health Services is seeking a Care Management Specialist to coordinate the RAI/MDS process, ensuring timely and accurate assessments and compliant documentation across Medicare/Medicaid programs.

You will collaborate with an interdisciplinary team to optimize care plans, manage MDS schedules, and ensure proper reimbursement while maintaining data integrity and regulatory adherence.

Qualifications

• Graduate of an accredited school of nursing with current RN or LVN license.
• Experience with MDS completion, reimbursement, clinical resource utilization and/or case management is desirable.
• Ability to lead interdisciplinary teams and coordinate the RAI process.
• Competency with standard office software and AIS training.

Responsibilities

• Coordinate the RAI process to ensure accurate and timely assessments.
• Collaborate with the interdisciplinary team to develop care plans and management.
• Ensure Medicare/Medicaid case-mix documentation accuracy and appropriate reimbursement.
• Maintain an accurate schedule of MDS assessments and reference dates.
• Perform concurrent MDS reviews to optimize RUGs and clinical documentation.
• Communicate with Regional Care Management Specialist and Business Office on case-mix and reimbursement.
• Audit MDS processes and validate data integrity.
