Nadia Care in Memphis is seeking a full-time, bilingual Maternity Navigator to support expectant and new mothers with care navigation, doula support, lactation assistance, nutrition, housing, transportation, and wrap-around services. You will bridge the gap between the healthcare system and the community, empowering mothers to access essential resources for a healthy pregnancy.

Qualifications

• 3+ years of experience in community health, patient advocacy, or healthcare navigation.
• Deep understanding of challenges faced by expectant mothers and experience providing prenatal and postpartum health education.
• Bilingual in English and Spanish.
• Excellent listening and communication with compassion and cultural sensitivity.
• Strong problem solving and ability to manage multiple priorities in a fast-paced field.
• Ability to work independently in the field while staying connected to a team of specialists.
• Valid driver’s license with a reliable vehicle and up-to-date registration and insurance.
• Access to home internet and a phone with data service.
• Tech-savvy and quick to learn documentation in AthenaOne, Welkin, and MacOS.

Responsabilités

• Meet with mothers virtually and in person to educate on urgent maternal signs, preeclampsia, and postpartum recovery.
• Educate members on birth positions and breathing techniques for confidence and preparation.
• Collaborate with a multidisciplinary care team and raise clinical issues during daily huddles.
• Maintain clear, accurate documentation of all care interactions for coordinated support.
• Build trusting relationships with mothers and provide support via call or messaging between visits.
• Connect mothers to community resources to overcome barriers like food insecurity, transportation, or housing.
• Provide insights from mothers to help shape Nadia Care programs to meet needs.
• Attend administrative meetings and follow Nadia Care policies to uphold service standards.
