Nadia Care is hiring a full-time Maternity Navigator to support expectant and new mothers in the Memphis area. You will serve as a trusted bridge between the healthcare system and the community, offering education, advocacy, and resource navigation to improve pregnancy outcomes.

Ideal candidates have 3+ years in community health or navigation, bilingual English/Spanish, and a compassionate, independent, yet collaborative approach. Remote-friendly and local field presence in South Memphis region.

Formación

• 3+ years in community health, patient advocacy, or healthcare navigation.
• Prenatal/postpartum health education experience.
• Bilingual in English and Spanish; strong listening and communication.

Responsabilidades

• Meet mothers virtually and in-person to educate on urgent maternal signs and postpartum recovery.
• Teach birth positions and breathing techniques to build confidence.
• Collaborate with a multidisciplinary care team and participate in daily huddles.
• Document all interactions clearly to ensure coordinated support.
• Build trusted relationships and provide guidance via calls or messages between visits.
• Connect families to housing, transportation, and food support as needed.
• Provide community feedback to shape Nadia Care programs.
• Attend administrative meetings and follow policies and procedures.
