Job summary

Nadia Care is hiring a Maternity Navigator to support expectant and new mothers in Montgomery and Prince George's County, MD. The role centers on bridging the healthcare system with the community, building trust, and delivering wrap-around support for a healthy pregnancy journey.

You will educate on health topics, connect to resources, and collaborate with a multidisciplinary team to ensure high-quality, coordinated care for families in the Maryland area.

Qualifications

• 3+ years in community health, patient advocacy, or healthcare navigation.
• Experience providing prenatal and postpartum health education.
• Excellent listening and communication with compassion and cultural sensitivity.

Responsibilities

• Meet mothers virtually and in-person, educating on warning signs, preeclampsia, and postpartum recovery.
• Educate on birth positions and breathing techniques to boost confidence.
• Attend daily care-team huddles and elevate clinical issues to specialists.
• Maintain clear, accurate documentation of care interactions for coordinated support.
• Build trusting relationships with mothers and provide support via calls or messaging between visits.
• Connect mothers to community resources to overcome barriers like housing or transportation.
• Provide insights from mothers to shape Nadia Care programs.
• Attend administrative meetings and follow policies to ensure high standards.
