Job summary

Nadia Care is seeking a full-time Maternity Navigator to advocate for expectant and new mothers in Rutherford County, Tennessee. The MN will bridge healthcare and community resources, delivering education and support to improve pregnancy outcomes while working within a multidisciplinary team.

Responsibilities include virtual and in-person visits, coaching on essential health topics, documenting interactions, and connecting families with social supports.

Qualifications

• 3+ years in community health, patient advocacy, or care navigation.
• Experience with prenatal and postpartum health education.
• Strong listening and communication with cultural sensitivity.
• Able to manage multiple priorities in a fast-paced environment.
• Independent field work with team collaboration.
• Valid driver’s license with reliable transportation.
• Access to home internet and a smartphone.

Responsibilities

• Meet with mothers virtually and in-person, educate on urgent maternal signs and postpartum recovery.
• Educate on birth positions and breathing techniques for confidence.
• Join a multidisciplinary care team and attend daily huddles with specialists.
• Maintain clear, accurate documentation of all care interactions for coordinated support.
• Build trusting relationships and provide compassionate, culturally sensitive support.
• Connect families with community resources to overcome barriers like housing or transportation.
• Provide feedback from mothers to shape Nadia Care programs.
• Attend administrative meetings and follow policies to maintain high standards.
