Job summary

Maternal and Family Health Services is seeking an RN Care Coordinator at our Scranton location to coordinate patient care across settings, provide direct nursing care, and counsel clients in line with agency protocols. The role supports maternity and family planning programs with emphasis on high-quality, equitable services.

The RN will work with a multidisciplinary team, maintain EMR documentation, and engage in education and counseling for patients in the program, ensuring access to necessary

Qualifications

• 2-3 years’ experience in OB/GYN preferred.
• PA-licensed nurse with current BLS certification.
• Ability to effectively interact and communicate with clients and staff.
• Demonstrate nursing process knowledge to identify needs and interventions.
• Ability to function independently and as part of an interdisciplinary team.

Responsibilities

• Provide nursing interventions in collaboration with the interprofessional team.
• Perform client assessments and develop a plan of care in collaboration with the team.
• Update the plan of care as needed based on client status.
• Assume responsibility for nursing care of each client.
• Provide client-centered counseling and education within MFHS programs.
• Review and maintain labs and EMR; alert providers of abnormal results.
• Notify clients of abnormal results and management.
• Document accurately in the EMR and ensure timely records.
• Refer clients to other resources and networking services as needed.
• Collaborate with clinical team members and support organizational goals.
• Represent MFHS in health centers and the community professionally.
