Job summary

Centers for New Horizons seeks a Maternal Health Navigator to support pregnant and parenting individuals with personalized navigation, case management, and community resource coordination in Chicago.

You will work with Family Connects and partners to improve maternal and infant health outcomes, while reducing barriers to care and promoting access to services across the Chicagoland area.

Qualifications

• Associate degree required in Social Work, Human Services, Public Health, Behavioral Science, Psychology, Sociology, Nursing, or related field.
• Bachelor's degree is preferred.
• Minimum of one year of experience in maternal and child health, public health, case management, family support, community outreach or related field.
• Experience with pregnant individuals, new parents, or underserved populations is strongly preferred.

Responsibilities

• Conduct in-person visits with pregnant and parenting clients across Chicago.
• Build trusting relationships and assess barriers to services.
• Provide navigation and case management to eligible participants.
• Connect families with healthcare providers, MCO resources, transportation, childcare, and other community services.
• Educate families about benefits and programs supporting maternal and child health.
• Promote healthy pregnancy and infant wellness through ongoing engagement.
• Advise families about Family Connects during prenatal period and referrals.
• Assist families with CCAP and other public benefits navigation.
• Deliver trauma-informed and culturally responsive services.
• Engage in outreach and community activities to raise service awareness.
• Maintain collaboration with community organizations and healthcare partners.
• Represent CNH at events and governance meetings.
