Job summary

CareSource is seeking a Maternity Care Coordinator to be the member concierge for the Mom and Baby Beginnings program. You will serve as the single point of contact for assessment, person-centered planning, and service coordination.

Responsibilities include enrolling members, conducting health surveys, educating families, documenting care, and coordinating with providers. Strong communication and cultural competency are essential in this role.

Qualifications

• Diploma or Associates in a healthcare related field or equivalent years of relevant work experience is required.
• Bachelor’s degree in a healthcare related field is preferred.
• A minimum of three (3) years of experience in a healthcare field is required (discharge planning, case management, care coordination, and/or home/community health management).

Responsibilities

• Telephonically engage and enroll members in Mom and Baby Beginnings program.
• Engage the member and complete a health and psychosocial survey, considering linguistic needs.
• Educate members on the Mom & Baby Beginnings program.
• Conduct initial screening and needs assessments.
• Verify eligibility, enrollment history, demographics, and current health status.
• Provide timely and accurate documentation.
• Build relationships by delivering quality customer service.
• Coordinate with assigned Physician offices to establish care.
• Document care coordination activities in the electronic record per standards.
• Suggest process improvements to enhance the member experience.
