Job summary

University Health is seeking an experienced registered nurse to coordinate interdisciplinary treatment plans for the maternal patient population. You will facilitate service delivery, assess effectiveness, track outcomes, and advocate for patients across the continuum of care.

Requirements include graduation from an accredited nursing program and a minimum of three years of clinical nursing. OB experience and EPIC EMR familiarity are preferred; bilingual English/Spanish is highly valued.

Qualifications

• Graduation from an accredited school of nursing is required.
• Minimum three years of clinical nursing experience is required.
• Data entry, referral management, and utilization management experience is required.
• Case management experience preferred.
• OB patient experience preferred.
• EPIC EMR experience desirable.
• Bilingual English/Spanish highly preferred.

Responsibilities

• Coordinates the interdisciplinary treatment plan of care for patients across the healthcare continuum within the maternal patient population.
• Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and advocates for patients.
• Identifies and communicates health care needs to patients and families.
• Collaborates with clinical staff, clinic leadership, and outside agencies to improve patient outcomes, compliance, and decrease complications.
