Job summary

Wisconsin Community Services is seeking a Maternal Health Care Coordinator in Milwaukee to provide intensive, individualized care coordination for pregnant and postpartum women. You will work with the participant, peer specialists, and a multidisciplinary team to develop personalized recovery plans addressing medical, housing, and psychosocial needs.

The role emphasizes trauma-informed support, strong communication, and timely documentation in the EMR.

Qualifications

High School Diploma or equivalent required.
1 year experience with mental health, substance use, and/or co-occurring needs.
Meet WCS and contractor requirements for background checks, driver’s record, and drug screen.
Valid driver’s license, automobile, and insurance to meet agency requirements.

Responsibilities

Provide one-on-one care coordination for pregnant women and new mothers.
Build trusting relationships and meet individuals where they are emotionally and physically.
Collaborate with care teams to develop, implement, and update individualized recovery plans.
Serve as a liaison with service providers to ensure coordinated care and referrals.
Monitor progress toward self-identified goals and provide supportive coaching and symptom management.
Document interactions in the EMR within 24–48 hours.
