Qualifications

• High School diploma or equivalent required.
• Two years college preferred.
• Home Health: 2+ years in customer service, preferably in home health care.
• Hospice: 2+ years in customer service, preferably in hospice.
• Maternal Health Clinic: AA or BA/BS in Social Work preferred.

Responsabilités

• Timely and accurate entry of patient information, and billing/payroll data.
• Responds to referrals from various sources.
• Data entry and system reporting to support patient care management.
• Communicates patient needs to clinical team via EMR.
• Oversees patient education and care preparation for home care.
