Responsibilities

• MAT support: engage MAT patients to promote adherence and retention.
• ECM: provide care coordination for high-need patients with a 13-person target panel.
• Develop and maintain comprehensive care plans addressing medical, behavioral health, and social needs.
• Coordinate care across primary care, behavioral health, and community services.
• Document in EHR and maintain program compliance and productivity targets.
• Conduct outreach, home visits, and system navigation to support clients.
