Job summary

Humboldt Neurohealth in Eureka, CA is seeking an on-site Case Manager to support Enhanced Care Management (ECM). Under the supervision of the Program Supervisor, you will provide direct service, coordinate care management, and connect members to medical and social services to improve outcomes.

You will assess client needs across health domains, develop and implement Health Action Plans, conduct outreach, arrange transportation, and maintain required documentation.

Qualifications

  • Must possess a valid California driver's license and personal auto insurance.
  • Must be willing to complete a personal background investigation.
  • Must be willing to work within a culturally integrated workplace and respect human differences.

Responsibilities

  • Assesses client needs in physical health; mental health; SUD; oral health; trauma-informed care; social supports; housing; vocational/employment; wellness; and referrals to services.
  • Oversees development and implementation of the Individual Care Plan/Health Action Plan.
  • Provides services where the client lives, seeks care, or is most accessible (office, telehealth, field-based).
  • Connects clients to social services and supports as needed.
  • Coordinates and advocates with health care professionals with consent (e.g., PCP).
  • Works with hospital staff on Transitional Care Planning.
  • Conducts outreach to facilitate linkage to ECM; coordinates transportation; accompanies clients to visits if needed.
  • Evaluates progress and updates goals; completes documentation on time; attends meetings and trainings.

Job description

Under the direction of the Program Supervisor, the Case Manager provides direct service, as well as coordinates care management and functions as a part of a "Care Team" for the Enhanced Care Management (ECM) Program. The Case Manager is responsible for providing coordinated care for patients to improve quality outcomes, reduce health disparities, and transform the delivery system through value-based initiatives, and modernization. Oversees provision of enhanced care management (ECM) services and implementation of the care plan. The Case Manager offers services where the member lives, seeks care, or finds most easily accessible and connects members to medical care and other social services the member may need.

ESSENTIAL RESPONSIBILITIES AND DUTIES include the following. Other duties may be assigned.

  • Assesses client needs in the areas of physical health; mental health; SUD; oral health; trauma-informed care; social supports; housing; vocational/employment; wellness; and referral and linkage to community-based services and supports.
  • Oversees the development and implementation of the Individual Care Plan/Health Action Plan.
  • Provides services where the client lives, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services.
  • Connects clients to other social services and supports that are needed (e.g., community support group).
  • Coordinates and advocates on behalf of the client with health care professionals with consent and authorization (e.g., PCP).
  • Works collaboratively with hospital staff regarding Transitional Care Planning.
  • Conducts outreach and engagement activities to facilitate linkage to the ECM program. Outreach and Engagement consists of phone calls, mailed information, and field visits.
  • Coordinates transportation for clients and accompanies them to office visits, as needed and appropriate.
  • Evaluates progress and updates goals.
  • Completes all required documentation within the timeframes established by the individual action plans.
  • Attends weekly staff/team meetings and supervision.
  • Attends training as assigned (e.g., ACEs Certification).
  • Completes other duties as assigned.
Requirements:

1. Must possess a valid California driver's license, personal automobile insurance and driving record that meets the standards outlined in the Agency's Policy: Motor Vehicle Operating Standards.

2. Must be physically and mentally fit in accordance with the Agency's Policy: Physical Fitness Standards and Examinations.

3. Must be willing to complete a personal background investigation conducted by the State of California.

4. Must be willing to work within a culturally integrated workplace and be willing to respect human differences based upon protected classes as defined by state, federal and local laws, or any other condition that distinguishes people from one another.
