Job summary

Hill Country Health & Wellness Center seeks a Mobile Crisis Outreach Team Case Manager to provide direct, culturally competent crisis response and case management in our community. You will collaborate with a Mental Health Clinician to assess safety, stabilize individuals in crisis, and link them to services and resources for recovery and long‑term wellness.

Qualifications

• Bachelor’s degree in social work or related field preferred; further education encouraged.
• Excellent communication skills; ability to read, write, analyze and interpret complex documents.
• Proficiency in Windows OS, MS Office, and EHR documentation.

Responsibilities

• Provide direct case management and recovery support services in a safe, professional, culturally competent manner.
• Safely initiate contact and establish rapport with callers and clients/potential clients.
• Assess crisis level and coordinate mobile crisis response; dispatch追加 team members when needed.
• Conduct mental status exams, crisis intervention, and first aid; de-escalate emotional states.
• Advocate for clients to access urgent psychiatric or health care, housing and benefits.
• Complete timely electronic documentation of interactions and services.
