Job summary

Hill Country CARE Center in downtown Redding is hiring a CARE Center Case Manager to work with clinicians in a walk-in crisis program. You will assess safety, coordinate resources, and develop action plans for urgent mental health and related needs, collaborating with a care team to promote stabilization and recovery.

You will document interactions within electronic health records, participate in weekly team meetings, and advocate for clients to access housing, benefits, and outpatient services.

Qualifications

• Bachelor's degree in social work or related field preferred; may substitute experience for education.
• Mental Health First Aid and related certifications preferred.
• Experience in crisis intervention and case management is beneficial.

Responsibilities

• Provide direct case management and recovery support services in a safe, professional way.
• Assess safety and mental status, provide crisis intervention and de-escalation.
• Develop goals and action plans with clients and coordinate resources.
• Document interactions in electronic health records within 24 hours.
• Collaborate with clinicians and team to ensure continuity of care.
• Advocate for clients to obtain urgent psychiatric or health care, housing, and benefits.
