Job summary

Hill Country Health & Wellness Center is seeking an experienced BHCC/ECM Case Manager in Round Mountain, CA. The role includes engagement with patients, facilitating access to community resources, and supporting complex cases on both medical floors and in the field.

Responsibilities include coordinating care, documenting thoroughly in the EHR, and assisting with enrollment and billing requirements for the ECM program. The position is full-time with day shifts.

Qualifications

• High School Diploma or GED; Bachelor’s preferred.
• 1–3 months related experience and/or training; 1–2 years preferred.
• Skill in communicating with patients and health professionals.
• Proficiency with EHR systems.

Responsibilities

• Assist patients in establishing self-management goals and enrollments.
• Provide health coaching and warm hand-offs from medical providers.
• Communicate care plan changes with patients and team.
• Track appointments and coordinate integrated behavioral health care.
• In-house and field case management, transportation, and advocacy.
• Document client transactions in the EHR for billing and records.
