Job summary

HealthRIGHT 360 seeks a Lead Care Manager to support Enhanced Case Management (ECM) within a Managed Care Plan. The role requires traveling to engage clients in various community and clinic locations, coordinating health, behavioral health, housing, and social services to address complex needs.

Responsibilities include intake assessments, care planning, and collaboration with probation, courts, and family services to support reintegration and continuity of care.

Qualifications

• High school diploma or equivalent required.
• Minimum of two years’ experience in the human service field; experience in substance use treatment or recovery preferred.
• Current First Aid and CPR certification.
• Valid California driver’s license and access to insured transportation.
• Experience with clients experiencing acute withdrawal from substances; trauma-informed services preferred.

Responsibilities

• Organize patient care activities and share information with the multi-disciplinary care team.
• Identify eligible individuals from state prisons, jails, hospitals for MCP enrollment.
• Engage clients via in-person visits, mail, email, texts, and outreach.
• Maintain regular contact with care providers to coordinate care and service input.
• Ensure integrated care across primary, behavioral, development health and SUD services; assist with transportation and appointments.
