Job summary

HealthRIGHT 360 is seeking a Care Manager for Enhanced Care Management (ECM) in San Mateo, CA. The role involves field-based outreach, coordinating physical and behavioral health services, and connecting clients with housing, transportation, and community resources.

Strong communication and documentation skills are essential. The candidate will work Monday–Friday, 8:00 am–4:30 pm, with a focus on meeting clients wherever they are, including shelters or their homes, to ensure a holistic approach

Qualifications

• High school diploma or equivalent is required.
• Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
• Valid California driver’s license and access to registered and insured transportation.

Responsibilities

• Enroll eligible individuals into ECM and engage them through multiple outreach methods.
• Coordinate client care per MCP requirements and share information with the care team.
• Maintain regular contact with providers and ensure integrated care across services.
• Meet ECM clients in homes or field settings and document activities efficiently.
• Complete progress notes within 24 hours and participate in supervision and training.
