Responsibilities

• Provide health education and coaching to members.
• Assist access to social determinants of health such as nutrition.
• Verify insurance coverage and benefits.
• Conduct utilization reviews for medical necessity.
• Collaborate with insurers and providers to ensure service quality.
• Serve as member advocate and address questions/concerns.
• Support community-based management of medication therapy.
• Maintain compliance with laws, regulations, and contracts.
• Document all interactions and communications accurately.
