You will coordinate referrals, transport clients, and maintain thorough documentation. Strong communication, confidentiality, and crisis de-escalation skills are essential to support clients with co-occurring disorders and families.

Qualifications

• High school diploma or GED required; associate’s or bachelor’s preferred.
• Experience with substance use disorders or mental health conditions is preferred.
• Strong communication and interpersonal abilities for client interactions.
• Ability to maintain confidentiality and adhere to HIPAA and state policies.

Responsibilities

• Assist clients with obtaining vital records and benefits such as Medicaid and SNAP.
• Coordinate referrals to community resources and support services.
• Transport clients to appointments using agency vehicles and document activities.
• Maintain documentation of case management activities per agency policies.
• Communicate with the treatment team about client progress and needs.
• Advocate for clients to ensure continuity of care and access to services.
