Job summary

Empower Healthcare Solutions, LLC. Little Rock, AR seeks an Entry Level Care Coordination Specialist to support foster care and pediatric adoption cases, ensuring services are delivered by specialty providers and continuity of care across the plan.

Responsibilities include high-touch outreach with families, coordinating with providers, developing PCSP, patient education, and addressing social determinants of health to improve outcomes.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in social science or health-related field preferred.
• 1+ years of experience caring for developmentally or intellectually disabled or behavioral health clients (Preferred).
• 6+ months of care coordination experience with pediatric members in foster care or adopted (Preferred).

Responsibilities

• Provide high touch outreach with assigned members and families.
• Wrap around services for adoption or child welfare journeys.
• Assist members with healthcare needs by ensuring providers and services work together to improve health.
• Provide health education and coaching tailored to treatment and service plans.
• Develop the Person-Centered Support Plan (PCSP) and provide choices for members.
• Ensure compliance with the PCSP and assist with resources or barriers to treatment.
• Coordinate with various healthcare providers for diagnostics and hospital services.
• Address social determinants of health including access to exercise and healthy food.
• Promote activities focused on patient health and community well-being.
• Coordinate community-based management of medication therapy.
