Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care as needed. This role serves as a clinical resource and can include a sign-on bonus.

The position requires licensure as PT (Illinois) or PTA/OT as applicable and may include variable shifts; a full-time schedule is anticipated. Responsibilities include patient transfers, education, documentation, and collaboration with the rehabilitation team.

Qualifications

• Licensed Physical Therapist (PT) per IDFPR requirements.
• BLS certification within 30 days by American Heart Association.
• Experience evaluating acute stroke patients and continuing education.

Responsibilities

• Perform safe patient transfers with proper body mechanics.
• Design and adjust care plans with goals and discharge plans.
• Educate patients and families using effective teaching methods.
• Collaborate with the care team and supervise PTA/rehab staff.
• Document in CTSCP67 compliant manner.
