Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients across the rehabilitation spectrum. The role supports plan of care development, patient education, and coordination with the care team in a dynamic clinical setting.

Lymphedema certification is preferred but not required; sign-on bonus is offered. The position may involve shift variations, with an emphasis on safety and quality outcomes for patients in our rehabilitative services.

Qualifications

• Requires degree in physical therapy and active PT license.
• Lymphedema certification preferred but not required.
• Current BLS certification is required within 30 days of hire.

Responsibilities

• Performs patient transfers with proper body mechanics to ensure safety.
• Develops and adjusts a plan of care based on patient response and outcomes.
• Educates patients and families using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and treatment results.
• Documents patient care according to CTSCP67.
• Coordinates with the rehab team and supervises PTA and rehab technicians.
• Ensures compliance with regulatory standards and quality initiatives.
