Job summary

Qualifications

• Bachelor's Degree in a relevant field.
• Licensed Physical Therapist (PT) or PTA credentialing considered.
• BLS within 30 days - American Heart Association.
• Ability to attend and complete continuing education for stroke care.

Responsibilities

• Demonstrates ability to transfer patients with assistance as needed, using safe body mechanics.
• Treats patients with empathy and respect, staying engaged with those needing services.
• Designs and adjusts a care plan with goals, treatment, outcomes, and discharge plan.
• Educates patients, families, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine diagnoses and outcomes of care.
• Performs examinations with reliable methods aligned to symptoms and history.
• Produces documentation per CTSCP67 policy and standards.
• Screens patients to determine therapy needs and progression.
• Utilizes support personnel within legal and ethical guidelines and provides feedback.
• Complete annual education to comply with program requirements.
