Job summary

Sweetwater Care in Windsor, CA is seeking an experienced LVN Nurse Manager to lead nursing service delivery in a long-term care setting. You will oversee clinical operations, mentor staff, ensure adherence to regulatory standards, and coordinate with physicians and departments to optimize patient care outcomes.

Ideal candidates have California LVN license, a compact license if applicable, 1 year of nursing management experience, and hands-on experience with long-term care and PointClickCare

Qualifications

• California LVN license is required; compact license if working in a compact state.
• Minimum of 1 year nursing management experience preferred.
• Experience in long-term care settings and charting with PointClickCare.

Responsibilities

• Provides nursing care within scope of practice per standards, orders, and policies.
• Participates in Quality Improvement Program and uses Quality Life Manual for patient placement.
• Notifies departments of patient changes and conducts family and patient education.
• Administers medications and treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by state practice act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
