Job summary

Sweetwater Care in Sonoma, CA is seeking a licensed LVN to provide nursing care within scope of practice, following physician orders, center policies, and state and local guidelines. The role emphasizes patient assessment, timely medication administration, and coordination with interdisciplinary teams.

Candidates should hold CA LVN license; long-term care experience and PCC charting familiarity are preferred. Management experience is a plus, though not required.

Qualifications

• Must be licensed LVN in California.
• One year of nursing management experience preferred but not required.
• Long-term care experience is required.
• PointClickCare charting experience preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of infection control procedures.
• Participates in and assists with the Quality Improvement Program.
• Uses the Quality Life Manual to identify, place, and monitor patients in programs.
• Communicates patient changes to other departments (room changes, LOA, new admits, discharge).
• Conducts family and patient education as needed.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by the Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state CA,
• One-year prior nursing management experience preferred, not required.
