Job summary

Sweetwater Care seeks an LVN supervisor in California to oversee nursing care within scope of practice and ensure adherence to physician orders and facility policies.

The ideal candidate will have California LVN licensure, experience in long-term care, and PCC charting knowledge to support patient care, staff supervision, and quality initiatives in a dynamic clinical setting.

Qualifications

• Must be licensed LVN in California.
• Nursing management experience preferred (1 year).
• Experience in long-term care settings.
• Experience with PCC charting.

Responsibilities

• Provide nursing care within scope of practice per standards and orders.
• Participate in quality improvement initiatives.
• Educate families and patients on care plans.
• Administer medications and treatments per policy.
• Receive and transcribe physician orders.
• Assist in rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
