Job summary

Sweetwater Care is seeking an experienced LVN to provide nursing care in a California facility. The role requires adherence to standards, physician orders, and facility policies while focusing on patient education and timely treatments.

The ideal candidate has current California LVN licensure, prior nursing leadership or management experience, and a background in long-term care with experience using Point Click Care charting.

Qualifications

• Currently licensed as LVN in state of California.
• One-year prior nursing management experience preferred, not required.
• Long term care experience.
• Point Click Care charting experience.

Responsibilities

• Provides nursing care within scope of practice in accordance with standards and orders.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders as permitted by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
