Job summary

Sweetwater Care is seeking a dedicated LPN with long-term care experience to join our clinical team. The selected candidate will contribute to patient care within the scope of practice and follow physician orders and facility policies.

Experience in nursing management is preferred, with strong knowledge of infection control and quality improvement processes. The role includes educating patients and families and coordinating care with physicians as needed.

Qualifications

• Currently licensed as LPN in state.
• One-year prior nursing management experience preferred, not required.
• Long term care experience.
• PointClick Care charting experience.

Responsibilities

• Provides nursing care within scope of practice to meet patient needs per standards and orders.
• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Uses Quality Life Manual for patient identification and monitoring.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Educates patients and families.
• Administers medications and treatments timely per facility policy.
• Transcribes and carries out physician orders as allowed by law.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.
