Job summary

Sweetwater Care seeks an LVN to provide nursing care under the physician's orders and center policies, ensuring patient needs are met. You will participate in quality improvement and use infection control guidelines while educating families and patients.

You will administer medications and treatments promptly, document orders per policy, and collaborate with physicians during rounds as needed.

Qualifications

• Licensed LVN in CA or with a compact license to practice in another state.
• Experience in long-term care settings preferred.
• Nursing management experience is a plus.
• Proficient with PointClickCare charting system.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Uses Quality Life Manual to identify, place, and monitor patients.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives and executes physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
