• Graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum of 2 years of experience in patient intake or medical office administration.

• Experience with data entry and administrative documentation.
• Strong organizational and communication skills.
• Ability to manage multiple priorities in a changing work environment.

Responsibilities

• Coordinate Home Health intake and care coordination.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Enter patient demographic information into required EMR systems.
• Audit new admission paperwork to ensure medical records are complete.
• Assist with obtaining authorizations for Home Health services and notify patients of financial responsibilities.
• Schedule patient visits and related Home Health services; coordinate IV therapy with vendors.
• Maintain confidentiality and comply with healthcare regulations; support the Home Health Manager and multidisciplinary team.
