Qualifications

• Graduate of an accredited School of Nursing is required.
• Current LVN license is required.
• Minimum of 2 years of experience in patient intake or medical office administration.
• Working knowledge of insurance plans and medical requirements.
• Proficiency with Microsoft Office and strong documentation skills.
• Ability to manage multiple priorities in a fast-paced healthcare setting.

Responsibilities

• Coordinate Home Health intake and care coordination to ensure timely service delivery.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Enter and audit patient demographics and medical records in EMR systems.
• Obtain authorizations and communicate financial responsibilities to patients and providers.
• Schedule patient visits and Home Health services; maintain patient communications.
• Maintain confidentiality and comply with healthcare regulations and policies.
• Collaborate with multidisciplinary teams and community referral sources.
