Job summary

TEEMA Group is seeking an organized LVN to serve as Clinical Intake Coordinator for Home Health operations. You will support referrals, authorizations, scheduling, and accurate medical records to ensure timely, high-quality care for vulnerable seniors.

Under supervision, you will collaborate with administrators, managers, providers, patients, families, and referral sources to maintain a smooth intake experience.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unencumbered Licensed Vocational Nurse (LVN) license.
• Minimum of 2 years of experience in patient intake or medical office administration.
• Basic working knowledge of insurance plans and medical requirements.
• Working knowledge of office management and filing systems.
• Experience with data entry and administrative documentation.
• Proficiency in Microsoft Office applications.
• Strong organizational and communication skills.
• Ability to manage multiple priorities in a changing work environment.
• Demonstrated ability to prioritize tasks in a fast-paced healthcare setting.
• Ability to work independently with minimal supervision.
• Strong attention to detail and commitment to accurate documentation.

Responsibilities

• Coordinate Home Health intake and care coordination.
• Receive referrals and coordinate orders for Home Health services.
• Manage the intake process with involved parties.
• Coordinate IV therapy treatments with vendors as needed.
• Schedule patient visits and related Home Health services.
• Collaborate with healthcare providers, the care team, Home Health staff, and other departments.
• Work with community referral sources to connect patients with services.
• Enter patient demographics into required EMR systems.
• Audit new admission paperwork to ensure medical records are complete.
• Enter new patient information into the EMR.
• Make welcome calls to patients and families to confirm information accuracy.
• Maintain medical records per federal, state, and county guidelines and organizational policies.
• Complete required documentation accurately and within timelines.
• Maintain filing systems and support efficient record management.
• Assist with obtaining authorizations for Home Health services.
• Notify patients about financial responsibilities and coverage.
• Respond to inquiries and provide exceptional customer service.
• Escalate concerns to management when needed.
• Provide clerical and administrative support to the Home Health Manager.
• Maintain relationships with the multidisciplinary team and other departments.
• Communicate workload, priorities, and needs with supervisors.
• Be flexible with assignments and hours as needed.
• Protect patient privacy and comply with regulations.
• Participate in quality improvement activities and mandated training.
• Be willing to work varied hours, including evenings, nights, weekends, and overtime.
