Under supervision of the Administrator and Home Health Manager, you will coordinate intake workflows, obtain authorizations, and collaborate with providers, patients, families, and community sources to deliver timely, high-quality care.

Qualifikationen

• Must be a graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum 2 years of experience in patient intake or medical office administration.
• Working knowledge of insurance plans and medical requirements.

• Coordinate Home Health services for patients in a timely manner.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Enter patient information into the EMR and maintain accurate records.
