Job summary

TEEMA Group is seeking an organized LVN to serve as a Clinical Intake Coordinator supporting Home Health operations in a patient-centered setting. This role focuses on coordinating patient services, managing referrals, and ensuring timely, high-quality care under the supervision of the Administrator and Home Health Manager.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum 2 years in patient intake or medical office admin.
• Proficiency with Microsoft Office and EMR systems.
• Strong organizational and communication skills.

Responsibilities

• Assist in coordinating Home Health services for patients in a timely and efficient manner.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Manage the intake process in collaboration with all involved parties.
• Coordinate IV therapy treatments with vendors as needed.
• Schedule patient visits, appointments, and related Home Health services.
• Collaborate with healthcare providers, the multidisciplinary care team, Home Health staff, and other departments to support patient care.
• Enter patient demographic information into required EMR systems.
• Audit new admission paperwork to ensure medical records are complete.
• Make welcome calls to patients and families to confirm the accuracy of information.
• Maintain medical records per federal/state/county guidelines and organizational policies.
• Complete required documentation accurately and within established timelines.
• Be willing to work varied schedule including evenings, nights, weekends, and overtime as needed.
