Job summary

TEEMA Group is seeking an organized LVN to serve as Clinical Intake Coordinator for Home Health operations. The role centers on coordinating patient services, managing referrals, and supporting providers to ensure timely, high-quality care.

You will supervise intake processes, schedule services, and maintain accurate medical records while collaborating with nurses, physicians, families, and referral sources. This is a full-time, on-site role with potential for varied hours to meet patient needs.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum of 2 years of experience in patient intake or medical office administration.
• Working knowledge of office management and filing systems.
• Experience with data entry and administrative documentation.

Responsibilities

• Coordinate Home Health services for patients in a timely and efficient manner.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Enter patient demographic information into required EMR systems and audit new admission paperwork.
• Maintain medical records in accordance with applicable guidelines and policies.
• Assist with obtaining authorizations for Home Health services and notify patients of financial responsibilities.
• Provide clerical and administrative support to the Home Health Manager and collaborate with care teams.
