Job summary

Under supervision, you will collaborate with providers, patients, families, and community sources to ensure timely, high-quality care and a smooth intake experience in a patient-centered environment.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum of 2 years of experience in patient intake or medical office administration.
• Basic working knowledge of insurance plans and medical requirements.
• Working knowledge of office management and filing systems.
• Experience with data entry and administrative documentation.
• Proficiency in Microsoft Office applications.

Responsibilities

• Coordinate Home Health intake and care coordination.
• Receive referrals and coordinate Home Health orders from providers.
• Manage the intake process in collaboration with involved parties.
• Coordinate IV therapy treatments with vendors as needed.
• Schedule patient visits, appointments and related Home Health services.
• Collaborate with care teams and referral sources to support patient care.
• Enter patient demographic information into required EMR systems.
• Audit new admission paperwork to ensure records are complete.
• Maintain medical records in accordance with regulatory guidelines.
• Be willing to work varied schedules including evenings, nights, weekends, and overtime.
