Job summary

TEEMA Group is seeking an organized, compassionate LVN to serve as a Clinical Intake Coordinator for Home Health operations. You will support referrals, authorizations, scheduling, and EMR documentation to ensure timely, high-quality patient care.

You will collaborate with nurses, physicians, patients, families, and community sources, handling intake workflows, data entry, and compliance with regulatory standards. Evening, weekend, or overtime hours may be required to meet patient needs.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum of 2 years of experience in patient intake or medical office administration.
• Working knowledge of insurance plans and medical requirements.
• Experience with data entry and administrative documentation.

Responsibilities

• Coordinate Home Health services for patients in a timely and efficient manner.
• Receive referrals and coordinate Home Health orders from providers.
• Manage the intake process with involved parties.
• Coordinate IV therapy treatments with vendors as needed.
• Schedule patient visits and related Home Health services.
• Enter patient demographic information into the EMR and audit admission paperwork.
• Maintain medical records in compliance with regulations and policies.
