Job summary

TEEMA Group is seeking an organized LVN to serve as Clinical Intake Coordinator for Home Health operations in Elk Grove, CA. The role supports referrals, authorizations, scheduling, and medical-record accuracy under supervision of the Administrator and Home Health Manager.

You will collaborate with providers, patients, families, and referral sources to ensure timely, high-quality care and a smooth intake experience. Evening/night/overtime flexibility may be required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unencumbered LVN license.
• Minimum of 2 years of experience in patient intake or medical office administration.
• Basic working knowledge of insurance plans and medical requirements.
• Working knowledge of office management and filing systems.
• Experience with data entry and administrative documentation.

Responsibilities

• Coordinate Home Health intake and care coordination in a timely and efficient manner.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Manage the intake process in collaboration with involved parties.
• Coordinate IV therapy treatments with vendors as needed.
• Schedule patient visits and Home Health services.
• Enter patient demographic information into required EMR systems.
• Audit new admission paperwork to ensure medical records are complete.
• Maintain medical records in compliance with regulations.
