TEEMA Group is seeking an organized LVN to serve as a Clinical Intake Coordinator for Home Health operations within a patient-centered healthcare organization.

You will coordinate referrals, schedule services, maintain accurate medical records, and collaborate with providers, patients, families, and community partners to ensure timely, high-quality care. Flexibility for varied hours may be required to meet patient needs.

Qualifications

• Graduate of an accredited School of Nursing.

Responsabilités

• Coordinate Home Health intake and care coordination in a timely manner.
• Receive referrals and coordinate Home Health orders from healthcare providers.
• Schedule patient visits and related Home Health services.
• Enter patient data into EMR and audit admission paperwork to ensure accuracy.
• Maintain medical records in compliance with federal, state, and local guidelines.
• Assist with authorizations and explain financial responsibilities and payer requirements to patients.
• Respond to inquiries from patients and providers with professional, compassionate communication.
• Provide clerical support to the Home Health Manager and collaborate with the care team.
• Be flexible with scheduling, including evenings, weekends, and overtime when needed.
