Job summary

Via Care Community Health Center in Los Angeles, CA is seeking a Licensed Vocational Nurse (LVN) to provide care coordination and support to Enhanced Care Management (ECM) members with complex medical and psychosocial needs.

The LVN will collaborate with ECM team members, primary care providers, and community partners, perform outreach, health education, and care planning, and ensure timely access to medical, behavioral health, and social services.

Qualifications

• Current, unrestricted California LVN license required.
• CPR/BLS certification preferred.
• Valid California driver's license and reliable transportation required.
• Minimum three years of clinical/care management experience.

Responsibilities

• Coordinate care for a caseload of ECM members with complex medical, behavioral, and social needs.
• Conduct outreach, engagement, and follow-up with ECM members and support systems.
• Educate and coach on chronic disease management, treatment plans, and medication adherence.
• Develop and update Shared Care Plans and Health Action Plans with ECM team.
• Document care plans and progress in the electronic health record.
• Participate in quality improvement activities and trainings.
