Job summary

BLEHEALTH is seeking a Lead Care Manager (LVN) to coordinate care for chronically ill members in collaboration with care teams. You will engage members, oversee ECM services, and implement care plans, with visits to member homes or facilities as needed.

Applicants must hold a California LVN license, demonstrate strong communication skills, and be able to navigate health IT systems. The role includes occasional in-person office presence and mileage reimbursement.

Qualifications

• Current LVN licensure in the State of California.
• Proficiency in communication technologies (email, cell phone, etc.).
• Experience with Health IT systems and reports is desirable.

Responsibilities

• Coordinate with members and providers to ensure seamless care.
• Engage eligible members and oversee ECM services and care plans.
• Provide services at member locations or where access is best.
• Refer and connect members to social services and transportation.
• Advocate for members with healthcare professionals.
• Use motivational interviewing and trauma-informed care approaches.
• Coordinate with hospital staff on discharge plans.
• Accompany members to office visits when needed.
• Monitor treatment adherence and health outcomes.
• Educate members and families for self-management.
