Job summary

University of Virginia Health is seeking a Post-Lung Transplant Coordinator RN to join the Lung Transplant Program in Charlottesville. You will be the central contact for patients after transplant, coordinating outpatient care and guiding them through recovery and long-term follow-up.

In this role, you will partner with transplant pulmonologists, nurse practitioners, and multidisciplinary teams to manage complex clinical situations, provide education, triage concerns, and support lifelong

Qualifications

• Licensed RN in Virginia.
• BLS certification required.
• Transplant nursing experience preferred.

Responsibilities

• Coordinate post-transplant outpatient care for lung recipients.
• Collaborate with transplant pulmonologists, NPs, and care teams.
• Triage rejection, infection, and medication issues.
• Educate patients and caregivers on recovery and follow-up.
• Monitor medically complex patients and arrange timely interventions.
