Job summary

UVA Health in Virginia seeks a dynamic transplant clinical nurse coordinator focusing on population management and relationship-based care within a multidisciplinary team. The RN develops, implements, and evaluates individualized care plans to educate, empower and optimize patient outcomes across all transitions within the continuum of care.

This role requires afterhours/weekend coverage as the primary contact for pre- and post-transplant patients and for internal and referring providers.

Qualifications

• BSN from an accredited program.
• 2 years of relevant experience.
• Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia with BLS certification. Board certification strongly preferred.

Responsibilities

• Provide afterhours/weekend coverage as primary point of contact for pre- and post-transplant patients and providers.
• Telephone assessments and complex care coordination based on comprehensive assessment.
• Coordinate with the multidisciplinary transplant team and keep members informed of clinical decisions and patient responses in a timely manner.
• Educate patients/families about transplant options, evaluation process, organ allocation, and post-operative care.
