Job summary

UVA Health in Virginia is seeking a dynamic transplant nurse coordinator focused on population management and relationship-based care within a multidisciplinary team. This RN develops, implements, and evaluates individualized plans of care to educate, empower patients, and optimize outcomes across all transitions of care.

The role requires afterhours/weekend coverage as the primary contact for pre- and post-transplant patients and for internal and referring providers.

Qualifications

• Bachelor of Science in Nursing from an accredited nursing program required.
• 2 years of relevant experience.

Responsibilities

• Provide afterhours/weekend coverage as primary contact for pre- and post-transplant patients and providers.
• Telephone assessments and complex care coordination based on comprehensive assessment.
• Educate patients and families about living donation, evaluation, organ allocation and transplant phases.
• Keep the transplant team informed of clinical decisions and patient responses.
• Participate in QAPI meetings and performance improvement activities.
• Promote transplant nursing through education and policy advocacy.
• Maintain transplant certification and professional memberships.
