Job summary

Humana Inc. is seeking a Transition/Custodial Prevention Specialist to work in the field with members to create care plans and coordinate services that promote health and independence.

The role emphasizes person-centered thinking and collaboration with care teams, including LTC and HCBS providers. Travel is required to visit members in the community, and the incumbent will work remotely with a 40-hour workweek, Monday through Friday, CST hours.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two years of experience with Medicare and Medicaid recipients, long-term care services, HCBS, or managed care.
• One or more years of professional experience with custodial and transitional care.
• Experience with electronic information applications/software including electronic medical records.
• Ability to travel regionally to meet members and families.

Responsibilities

• Provide specialized support for LTSS and address health-related social needs.
• Build relationships with facility staff and advocate for member care.
• Develop plans of care and identify LTSS providers and community partnerships.
• Coordinate services to meet member goals and empower independence.
• Educate members to maintain Medicaid eligibility and navigate benefits.
• Enter annual Level of Care assessments into the state portal.
