Job summary

This field role involves home visits, nursing facilities, or meetings at the member's location. You will support LTSS, address health-related social needs, collaborate with care teams, and educate on Medicaid eligibility.

Qualifications

• Must reside in Illinois.
• Bachelor’s degree in social sciences, social work, human services, or related field.
• Two+ years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care organizations.
• One+ years of custodial and transitional care experience.
• Experience with electronic medical records (EMR) systems.

Responsibilities

• Work one-on-one with members to create personalized care plans and coordinate services.
• Empower members to achieve better health outcomes and independence.
• Support members residing in nursing facilities and coordinate with facility staff and care teams.
• Advocate for member care, including access to behavioral health services.
• Develop and modify Individual Service Plans with the care team and LTSS providers.
• Educate members to maintain Medicaid eligibility and navigate HCBS.
